Select the Child Id radio button and complete the following information to perform a search by child identification number:
In Child Id, enter the specific child identification number on which to search.
Select the Authorization radio button and complete the following information to perform a search by authorization number:
In Authorization #, enter the specific authorization identification number on which to search.
Select the Child Name radio button and complete the following information to perform a search by child name:
In Last Name, enter the last name of the child on which to search.
In First Name, enter the first name of the child on which to search.
Select the Date Range radio button and complete the following information to perform a search by date:
In Type, select the type of date on which you want to search.
In Start Date, enter the starting date of the date range on which to search.
In End Date, enter the ending date of the date range on which to search.
Select the Provider radio button and complete the following information to perform a search by provider:
In Last Name, enter the last name of the provider on which to search.
In First Name, enter the first name of the provider on which to search.
Click the following button(s) as necessary:
Click Back to return to the previous page.
