The following information is displayed for each authorization:
Authorization # displays the file identification number of the authorization.
Authorization Status displays the status of the authorization number.
Service Type displays the type of service provided.
Dates displays the authorized start date.
To displays the authorized end date.
Authorization Type displays the type of authorization.
Eff. Cancel Date displays the effective date of cancelation.
Acct displays the provider account number.
Provider displays the name of the provider account authorized to deliver services.
Print Date displays the date on which the authorization was initially printed.
Service Coordinator displays the name of the care or ongoing service coordinator for the child.
Phone displays the phone number of the care service coordinator.
In the columns of the Authorization Line table, view the following information for each line item associated with a authorization:
EI Code displays the local code for service and transportation authorizations.
HCPCS Code displays the HCPCS code for Assistive Tech authorizations.
Description displays the description of the authorization.
No. of Occurrences displays the number of times the service is provided.
Frequency displays the frequency with which the service is provided.
Duration displays the number of minutes that each occurrence of the service lasts.
CPT provides a link to the CPT page which lists the CPTs associated with the EI Code for the authorization.
Click the following button(s) as necessary:
Click Print to print the current authorization on your printer.
Click List Claim to display the Claim List page which contains a list of all the claims submitted for the authorization.
Click Claim Entry to enter a new claim for the authorization.
Click Back to return to the previous page.
