In the columns of the Provider List table, view the following information for each provider that is displayed:
Select provides a link to select this provider for the AT authorization. Click the link in this column to select the provider and return to the New AT Authorization page.
Last Name displays the last name of the provider.
First Name displays the first name of the provider.
Provider Account # displays the provider's account number.
Specialty displays the provider's specialty.
Billing Entity displays the provider's billing entity.
Parish displays the name of the parish in which the provider is located. Point to the icon in this column to view the provider's parish information.
