Search Provider

Use this page to perform a search and filter the list of providers that display on the Provider List page.

Tip:  Some of the fields on this page will allow the entry of partial information to help you narrow your search.

Performing a Search by Provider Account ID

In Provider Account ID, enter the specific provider account identification number on which to search.

Performing a Search by Provider Name

Complete the information in one or more of the following fields to search for a provider by name:

• In Provider Last Name, enter the last name of the provider on which to search.

• In Provider First Name, enter the first name of the provider on which to search.

Performing a Search by Billing Entity Name

In Billing Entity Name, enter the name of the billing entity on which to search.

Performing a Search by Zip Code

In Zip, enter the Zip code on which to search.

Performing a Search by Parish

In Parish, select the parish on which to search.

Performing a Search by Specialty

In Specialty, select the specialty on which to search.

Click the following button(s) as necessary:

• Click Back to return to the previous page.

• Click Search to perform a search based on the filter criteria entered and display the results on the Provider List page.
